Ilam Hospital says Dr Govinda KC's life is at risk on fourth day of hunger strike

His blood sugar fell to 36 milligrams per decilitre, Setopati reported, and Koshi's health secretary said no demand is the province's to meet.

Ilam Hospital has described the condition of Dr Govinda KC, the veteran campaigner for cleaner medical education in Nepal, as life threatening four days into his 25th hunger strike, OnlineKhabar reported on Sunday, November 1. Medical superintendent Dr Amitabh Thakur said it had reached a critical level. Nagarik reported doctors warned his condition could become dangerous.

KC began the fast on Thursday, October 29, at Narayan Temple in Ilam, demanding that the government scrap a decision letting Shahid Dasharath Chand University of Health Sciences in Geta start MBBS and nursing programmes without the staff or infrastructure the law requires, and that it enforce a rule reserving at least 75 percent of seats at publicly funded medical colleges for scholarship students, Nagarik and OnlineKhabar reported. Among other demands, he also wants a cancelled government medical college in Bardibas, Madhesh Province revived, and new public colleges built in Ilam, Udayapur and Dadeldhura, the outlets said. That 75 percent rule decides how many seats at publicly funded colleges go to scholarship students. The Education Ministry says these scholarship quotas let a large number of students study free of cost, OnlineKhabar reported.

His condition worsened after he was admitted to Ilam Hospital around 8 pm on Saturday, Setopati reported. Tests showed his blood glucose falling to 67, 67, 47 and then 36 milligrams per decilitre, a dangerously low reading for a person with type 2 diabetes, Setopati reported, citing hospital data. His blood pressure stood at 100 over 60, his blood oxygen between 92 and 96 percent, and an ECG occasionally showed abnormal heartbeats, Setopati reported. Dr Mahesh Acharya, one of the doctors treating him, said KC's blood sugar had dropped sharply and a urinary tract infection had worsened, OnlineKhabar reported.

Thakur said the hospital tried to give KC glucose but he refused it. "We are trying to give him glucose, but he has not agreed," Thakur said, according to Nagarik. "This is a very risky situation. Without glucose, it will be very difficult for him. We have already informed the authorities above about this."

The hospital has written to the Ministry of Health and the Koshi Province government seeking help, OnlineKhabar reported. Nagarik and Setopati reported the hospital asked the Koshi Province health ministry to arrange further treatment. None of the cited reports said KC had been moved out of Ilam Hospital as of Sunday. Koshi Province Health Secretary Dr Kritipal Subedi visited Ilam Hospital on Sunday to meet Thakur and the treating doctors, Setopati reported. Subedi said none of KC's demands could be addressed within the provincial government's authority, though he would support efforts to meet them, Setopati reported.

KC has argued that keeping the Geta teaching hospital running below capacity forces patients in Sudurpaschim Province to travel to Nepalgunj, Kathmandu or India for treatment, OnlineKhabar reported.

What the coverage missed

KC says his current demands repeat promises the government made before and did not keep. The Education Ministry says it is implementing them. Nepal's Education Ministry said in a statement, reported by OnlineKhabar, that task forces are "actively working" to implement the latest agreement reached with KC. Separately, Khabarhub reported that as he announced his 24th hunger strike earlier this year, KC accused the government of failing to act on a legal requirement to make medical education fully non profit by 2028.

KC has also alleged, in a statement reported by OnlineKhabar, that the Medical Education Commission, the government body that runs the common medical entrance exam, sets seat quotas and regulates colleges, has a vice chairperson who previously served as principal of Nepal Army Medical College before that college's seats were increased, and that a relative of the vice chairperson was separately appointed to a committee that decides college quotas. OnlineKhabar reported the allegation as KC's own claim. No official inquiry into it has been confirmed, and the vice chairperson has not been named or given a chance to respond in these reports.

What we still do not know

  • Whether KC has been moved from Ilam Hospital since Sunday, after the hospital asked the Koshi Province health ministry to arrange further treatment.
  • Whether the Ministry of Health or Ministry of Education has acted on Ilam Hospital's request, a question for those ministries' spokespersons.
  • Whether any body, such as the Commission for the Investigation of Abuse of Authority or the Medical Education Commission itself, has opened an inquiry into KC's conflict of interest allegation.
  • KC's clinical status beyond Sunday's hospital bulletin, which only Ilam Hospital can update.

Editorial review record

Why it ran today

  • ranking judge 1: ranked #1 with 80/100. This broke today and is confirmed by OnlineKhabar, Setopati and Nagarik, with Ilam Hospital's superintendent on record calling it 'life-threatening'. KC's demands on medical seats and scholarships reach students' families. Weakest dimension is personal stakes. Keep the hospital's wording attributed, credit the 36 mg/dL figure to Setopati alone, and recheck his condition and any ministry response before writing.
  • ranking judge 2: ranked #1 with 80/100. Confirmed. A life at risk inside the window, reported by OnlineKhabar, Setopati and Nagarik, with Ilam Hospital's superintendent Amitabh Thakur on record, so it meets the breaking call and passes the tag test. Weakest dimension is personal stakes. Recheck whether he was moved after Ratopati's referral report, and credit the 36 mg/dL reading to Setopati alone.

What the desk dug up

  • The pattern. KC's current six demands largely repeat unmet promises from his 24th strike and an August 27, 2026 government agreement, continuing a 13-year cycle in which successive governments have pledged reforms during his hunger strikes and then not fully implemented them. [OnlineKhabar English] [Khabarhub English]
  • Who benefits. KC himself has publicly alleged a conflict of interest at the heart of the Geta seat-increase decision: the Medical Education Commission's current vice-chairperson previously served as principal of Nepal Army Medical College, which was among the institutions granted additional seats under the same decision KC is fasting against. [OnlineKhabar English] [The Kathmandu Post]
  • Left out. Among KC's six demands is reversing the government's decision to scrap the Bardibas Medical College project in Madhesh Province -- a publicly budgeted project estimated at roughly Rs 30 billion that has sat stalled since at least fiscal year 2016-17 -- a concrete, quantifiable public-investment dispute missing from today's clinical coverage. Judge 1: The body is carefully sourced, but it ignores the ranking order to recheck whether KC was moved. It also states the hospital's life threatening call flat on the card and headline, and mistranslates Setopati's Subedi quote as 'most' when he said none of the demands fall within provincial authority. An unsourced claim that the Health Ministry has not responded, a contested agreement date and weak personal stakes are all fixable in one pass.

Judge 2: All five edits from the last ruling were made: social_hook, caption hook, the Geta sentence in the caption, the scholarship sentence now sourced to the Education Ministry via OnlineKhabar, and the Ministry of Health letter credited to OnlineKhabar alone. A full reread against the pages turned up no new unsourced claim, no changed name, date or number, and no hedged claim stated flat. The ranking orders are met: the hospital's wording is attributed, the 36 mg/dL reading is credited to Setopati alone, and the unknowns list keeps the questions about a transfer and a ministry response open. One small style lapse remains and does not block publication: the caption's Setopati glucose sentence runs 23 words, over the 20-word limit.

The complete record, including the advocate, skeptic, red team and defence arguments, is kept by the newsroom.
